Patients who have commercial insurance that do not cover the full costs of TearCare® may be eligible to receive a rebate to help with their out-of-pocket expenses. Patients are responsible for complying with their commercial insurance plan requirements (e.g., co-payments) when applying for and accepting the TearCare Rebate. Patients shall not seek reimbursement for the value of the TearCare Rebate amount received from this program from any other third-party, such as their health insurance program or plan.
By submitting this TearCare® Rebate form, you are confirming that you are not a federal health care program beneficiary or eligible for Medicaid (including Medicaid managed care), Medicare (including Medicare gap or Medicare Advantage), TRICARE, Veterans Affairs, or similar state or federal programs. The TearCare® Rebate Offer is prohibited for minors, and is void where prohibited by law, or otherwise restricted. The TearCare® Rebate is limited to patients who are residents of the United States and Puerto Rico.
The TearCare® Rebate offer is available to patients who have paid out-of-pocket to cover the cost of TearCare® in the United States. Sight Sciences, Inc. is not responsible for lost, late, damaged, illegible, misdirected or postage-due submissions. The TearCare® Rebate offer cannot be assigned or transferred. Submitting false information or seeking reimbursement for expenses covered by a third-party could constitute fraud and may result in federal prosecution under applicable federal and/or state laws (e.g., the US mail fraud statutes, Title 18, USC 1341 and 1342).
Call 1-888-580-8286 weekdays, 8am to 4:30pm CT if you do not receive your check within 90 days of the postmark date on your qualifying rebate application. Sight Sciences, Inc., reserves the right to rescind, revoke or amend this TearCare® Rebate offer without notice.
